Practice: Write an Objective Session Note
Read the scenario, write a brief note (4–8 sentences), then click Evaluate. The tool will flag subjective wording and highlight objective data.
Scenario
RBT arrives to the family’s home at 3:00 PM. The client (age 6) is finishing a snack and watching a short video. After a 2-minute greeting and clean-up, the RBT transitions to the work area. The first half hour focuses on the PECS Phase III program using small animal figures. Across the work block, exactly twelve exchange opportunities are presented. The client selects the correct picture and completes the exchange on ten of those opportunities; two errors occur when the distractor card is placed closer than the target.
At about 3:45 PM, the RBT announces a switch from table to a brief gross-motor break and then back to table. When the table demand is re-introduced, the client drops to the floor twice and cries (audible vocalization, no words) for roughly three-quarters of a minute. The RBT follows the Behavior Intervention Plan: planned ignoring of the crying and differential reinforcement of compliance. Crying ends within one minute, and the client returns to task following a single verbal prompt.
For the final segment, the RBT runs a two-step direction program using simple play tasks. Ten trials are presented; the client completes seven independently and requires a verbal prompt on three. The client’s parent joins near the end, observes for fifteen minutes, and completes three coached practice trials. Session wraps at 5:00 PM with materials put away and a quick schedule preview for next time.

Targets: PECS Phase III; two-step directions. Replacement skill: functional communication.
Programs/Data:
- PECS: 12 exchange opportunities; 10/12 correct (83%). Errors occurred when the distractor was closer than the target.
- Two-step directions: 10 trials; 7/10 independent (70%); 3 with verbal prompt.
Behavior Observed:
At 3:45 PM when table demand was reintroduced, client cried ~45 seconds and dropped to the floor twice. No aggression or SIB observed.
Intervention Implemented & Response:
Behavior Intervention Plan followed (planned ignoring; differential reinforcement of compliance). Crying ended within 1 minute; client returned to task after one verbal prompt.
Parent/Caregiver Involvement:
Parent observed ~15 minutes at the end and completed 3 coached practice trials.
Plan/Next Steps:
